3. In your view, what does not work well in the system and what would make it better?

The Review has been asked to "focus on improving the efficiency, effectiveness and trauma-informed nature of investigations" and "identify the specific barriers that families face in engaging with the process and the timescales involved, identifying any improvements that can be made on both fronts".

Please tell us about as many issues as you wish, whether you have suggestions for fixing them or not.

If you do have suggestions for changes, explaining to us what is wrong first will help us understand those suggestions.

If you are aware of examples of better approaches to any of the issues you have observed or experienced, please let us know what they are. This does not have to be taken from another death investigation process – it could be from a completely different experience.

5. Do you have any comments to make on reporting on Fatal Accident Inquiry recommendations or on the way that Fatal Accident Inquiries are done?

Please tell us what you think about reporting on FAIs, for example whether there should be

  • reporting on how the process is managed overall (how many FAIs are done, how long they take)
  • reporting on outcomes of FAIs (what they recommended, what has been done about those recommendations)
